Not sure how to respond but trying something.  yes we did see the doctor and everything is fine structurally or so he says. He told us to use the 2% xyoclaine get. It does not seem to work.</p>

Like Benzocaine and xylocaine. If not the you all should be fine but you need to find out why there is pain. The pain is there telling you all that something is not right and needs to be looked at. However, the bigger question is why are you having so much pain with intercourse? That is definitely not normal, so just numbing the area is not a satisfactory solution. You need to find a doctor that will help you find the cause of the problem. You didn&#8217;t say if the doctor was an OB/GYN or a GP. Start with an OB/GYN. If he/she can&#8217;t help, you may need to find one that specializes in sexual dysfunction.</p>

<p><b>A: </b>Symbicort is not a rescue inhaler.  Symbicort is a combination product containing Budesonide + Formoterol.  Budesonide is an inhaled corticosteroid and Formoterol is a long acting Beta 2 agonist (LAB2A).  To take Symbicort, take 2 inhalations twice a day.  This medication is indicated for individuals 12 years old and older.</p>
<p>Advair 500/50 is also a combination product containing Fluticasone + Salmeterol.  Fluticasone is an inhaled corticosteroid and Salmeterol is a LAB2A.  This medication is also taken twice a day and is indicated for children 4 years or older.</p>
<p>ProAir is a rescue medication.  It is a short acting Beta 2 agonist.</p>
<p>More likely, Symbicort was substituted for Advair not ProAir.  Check with your doctor or pharmacist.</p>
<p><b>Q: </b>Can I continue using my symbicort inhaler past the expiry date?<br />I have very mild asthma. I have been prescribed symbicort to use when I have problems which is very rare &#8211; seemingly only when I get a cold once or twice each year. </p>

<p><b>A: </b>Simply read the instructions written by the pharmacist on the package the medications came in. Use the nebulizer as directed, 2 or 3 times a day. Take the Symbicort as directed, usually twice a day. You could take the Symbicort after the nebulizer treatments for maximum effectiveness. Of course if the Symbicort is only ordered twice a day and the nebulizer is ordered three times then it would be best to use the Symbicort after the first and last treatment.<br />
The inhaler,  you should carry with you all the time. This is for emergency use like if you&#8217;re at work and you get short of breath you take a couple of puffs of the inhaler for relief.<br />
By using your medications properly you should have fewer and fewer problems.<br />

<p><b>A: </b>Maybe you developed a inhalational sensitivity to the powder .. I am not sure why this happens &#8230; but there is some anectdotal evidence that this happens &#8230;.</p>
<p>You may want to try taking your rescue inhaler first &#8230; then taking your symbicort 15-20 minutes later &#8230;. this maybe just that your airways are a little more &#8220;twitchy&#8221; than usual &#8230;.. you may want to make a visit with your doctor and discuss your concerns and findings&#8230;..</p>
<p>My overriding concern is that you do not take your medicine and if your on a fringe of a severe bronchial hyperresponsiveness episode, this may tip you into a hospital admission or an ER visit &#8230;. not saying that it will &#8230; but .. better safe than sorry?</p>

<p><b>A: </b>Yes, call the poison control center in your area.  If not that then go to the ER.  Salmeterol overdose can cause rapid heart rate, hypertension, agitation, etc&#8230;.<br />
They may need to monitor your vital signs for the next few hours, and administer beta blockers in a controlled setting if necessary.  Also, make sure you understand which inhalers are for rescue and which are for maintenance.  Maintenance inhalers aren&#8217;t useful for acute attacks and only lead to overdose as a patient keeps trying to get them to work quickly.<br />
Salmeterol is long-acting by design so the effect are going to be there for a number of hours.  So, don&#8217;t  ignore it.</p>

<p><b>A: </b>1, 2. Yes, I am on the pill Singulair. I know I was on some kind of steroid inhaler before, but the name escapes me.</p>
<p>3, 4 Yes, and technically I shouldn&#8217;t answer this question because I *used to* be on Advair. It didn&#8217;t do much for me, and because of the hypothetical risks, and cost, we dropped it. My Brother The Doctor (family practice) only recommends Advair if other stuff doesn&#8217;t work.</p>
<p>5. There is no real test for asthma. If someone has the symptoms, you treat it like asthma, if you get better, Bingo!</p>
<p>6. Shortness of breath and wheezing, usually induced by: (a) exercise and (b) certain allergies (dust, pollen, something blooming every spring). </p>
<p>It can progress to a persistent (like for months) cough, when that gets bad I can&#8217;t sleep at night unless propped up. </p>
<p>The symptoms get worse when I lie down, better with warm, moist air.</p>
<p>Since I monitor myself, I&#8217;ve never had to have a full fledged attack, where I couldn&#8217;t breath and have to get taken to the hospital.</p>
<p>7. I have Albuterol for treating the symptoms/when things get bad. I take Singular daily as prevention. As a side note, I also drink lots of water when I am symptomatic. It seems to help loosen things up in my chest. </p>
<p>Also &#8211; my pulmonologist said to avoid cough suppressants whenever possible. He said they just make it harder to get the stuff out of your lungs (my simplification). I do take expectorant when I&#8217;m coughing, it does help.</p>
<p>There is a lot of information on how to live well with your asthma, the National Institute of Health link below could be a good starting point.</p>

She asked me if I still use my becotide inhalers (steroidal 2 puffs morning and night) and I answered honestly that i didnt all the time because sometimes I forget. So she took it upon herself to change my medication and now i get seretide 125 which is &#8217;salmeterol/fluticasone propionate&#8217; (instead of salbutomol and becotide). I was told to take this 2 times in morn and 2 times at night. I CANNOT now get my salbutomol inhalers because the surgery says I have these new inhalers instead. BUT being chronic asthmatic I get tight chest throughout the day so I have to use the new seretide 125 inhalers more than 10 times a day (luckily they have same effect of stopping my breathlesness). I usually get 2 inhalers on repeat prescription (salbutomol has 200 metered doses *2) but the new inhalers have only 120 metered doses each so effectivelly the nurse has almost halved the amount of medication I get with each scrip. I ask for a repeat scrip when im on my last inhaler and now the surgery is saying im ordering my scrips too early and yesterday only prescribed me 1.</p>

<p><b>A: </b>There is a worrying collection of problems in your long question, I will try and give some simple, general answers.</p>
<p>Firstly if you GP gives his nurse the OK, she can change medication, ideally in line with protocols he has constructed, BUT he must sign the prescriptions and he retains overall responsibility for her actions.</p>
<p>Secondly, there is a move to put increasing numbers of symptomatic asthmatics on combi steroid/long acting beta 2 inhalers to improve control and compliance. ( I prefer Symbicort to Seretide.) There is also a licence for Symbicort, rather than Seretide to be used twice daily and if required if the patient is symptomatic. I am unhappy about this arrangement as it allows rather uncontrolled use of quite potent steroids. ( Particularly Fluticasone, which in high dose, has even been associated with death due to adrenal suppression!) Also there is no reason why a patient on a combi inhaler cannot also be on a reliever such as Ventolin/salbutamol as well.</p>

<p><b>A: </b>Yeah am I nurse in the hospital field and field questions like this all time.  The drug companies have to release information like this in the interest of full disclosure.  This is a medication and the results of something going wrong are there.  In the reality it&#8217;s like a 1 in million or so chance that something can happen to you, but with even that slim margin and the lawyers we have to do they want to tell you up front.  It&#8217;s like a blood transfusion at the hospital.  Yes there is a small, very small, chance that you could contract aids, hepatitis, or even die from the transfusion.  But on the other hand the is a very good chance that you will die if you don&#8217;t get the transfusion.  Medicine is almost the same way.<br />
While yes there is a small, very small, chance that you could suffer from a serious respiratory illness.  It&#8217;s a much greater chance that with out you will also suffer a respiratory illness.  </p>
<p>It&#8217;s like my blood pressure medication.  Yeah there is a small chance it could make me bottom my blood pressure.  But there is a much greater chance of my having a stroke without it.</p>
<p>I have given Advair several times and have not seen  any of the adverse reactions that they list in the papers.</p>

<p><b>A: </b>Don&#8217;t worry about it.  I have been taking that discus for almost 3 years now, twice a day, everyday.  What it means is that there is a slight risk because it is a steroid, but it is quite safe.  The only thing you do NEED to do is rinse your mouth out after using it, because the powder you inhale could cause thrush (a mouth infection).  Don&#8217;t worry, it is great and has really helped my asthma a lot!  Good luck to you!</p>

<p><b>A: </b>It basically means that not everyone can use this medication. Some people may have an allergic reaction to one of the ingrediants. Some people may find the drug does not help their asthma.</p>

<p>What&#8217;s more important with advair (which I&#8217;ve taken for years) is that you shake it well, hold it level, breathe in, do NOT breathe out into the canister, and rinse your mouth afterwards.</p>

<p><b>A: </b>1, 2. Yes, I am on the pill Singulair. I know I was on some kind of steroid inhaler before, but the name escapes me.</p>
<p>3, 4 Yes, and technically I shouldn&#8217;t answer this question because I *used to* be on Advair. It didn&#8217;t do much for me, and because of the hypothetical risks, and cost, we dropped it. My Brother The Doctor (family practice) only recommends Advair if other stuff doesn&#8217;t work.</p>
<p>5. There is no real test for asthma. If someone has the symptoms, you treat it like asthma, if you get better, Bingo!</p>
<p>6. Shortness of breath and wheezing, usually induced by: (a) exercise and (b) certain allergies (dust, pollen, something blooming every spring). </p>
<p>It can progress to a persistent (like for months) cough, when that gets bad I can&#8217;t sleep at night unless propped up. </p>
<p>The symptoms get worse when I lie down, better with warm, moist air.</p>
<p>Since I monitor myself, I&#8217;ve never had to have a full fledged attack, where I couldn&#8217;t breath and have to get taken to the hospital.</p>
<p>7. I have Albuterol for treating the symptoms/when things get bad. I take Singular daily as prevention. As a side note, I also drink lots of water when I am symptomatic. It seems to help loosen things up in my chest. </p>
<p>Also &#8211; my pulmonologist said to avoid cough suppressants whenever possible. He said they just make it harder to get the stuff out of your lungs (my simplification). I do take expectorant when I&#8217;m coughing, it does help.</p>

<p><b>A: </b>1) Salmeterol (Serevent) is not a rescue medication. Because it takes about a half hour to take effect, if you use Salmeterol for an asthma attack, it won&#8217;t work like an Albuterol inhaler would. When you&#8217;re having an asthma attack, never take Advair to treat it. Use your rescue inhaler. Inhaled steroid, like the Flovent in Advair, is also not a rescue medication.</p>
<p>2) What this means is that Salmeterol is not for use as a rescue medication. If someone was to use it like that, it might not work fast enough to do anything about an asthma attack. Therefore, taking Salmeterol for an asthma attack is almost as bad as doing nothing about it. Using your rescue inhaler is recommended.</p>
<p>3) If you took Advair to treat an asthma attack, you might not get relief from it. </p>
<p>4) Bronchodilators (like Serevent and Albuterol) do have a potential side effect of bronchoconstriction/bronchospasm. In my experience, I haven&#8217;t seen it happen yet. Sometimes a stronger bronchodilator might be needed, if Albuterol isn&#8217;t strong enough for rescue. Xopenex is often used in that situation. If you knew that your symptoms would worsen when using a medication, logically I would not use it. Getting a doctor&#8217;s appointment and asking about switching to something else is a good idea if that happens.</p>
<p>5) If you take your medication and nothing happens, that&#8217;s a good sign that your asthma is getting worse or your medication is losing effect (it happens rarely). Generally, I think that worsening of breathing and asthma attacks are similar enough to warrant the same treatment.</p>
<p>I was on Advair 500/50 for a few months. I found that it was safe and effective. One thing I noticed was that I had little spots of blood on tissues when I blew my nose. It was a bit weird, but actually is a side effect. </p>
<p>When taking an inhaled steroid, it&#8217;s extremely important to gargle/rinse your mouth after you take it, then spit the water out. I&#8217;ve seen some patients I treated swallow the water after rinsing and I did it too, for a short time. My stomach was upset by this. I don&#8217;t recommend swallowing the rinse water.<br />
Not doing this can lead to thrust (fungus on your tongue) developing. It&#8217;s something that you can also combat by brushing your tongue and using mouth watch.</p>
<p>It sounds like you&#8217;ve been reading the product manuscript or the information form that came from your pharmacy. Good job! Informed patients are a good thing. What you need to understand about the information you were provided with is that severe side effects are rare or happen when the medication is not used properly. I would keep in mind what the side effects are, so that you will know if it&#8217;s related to what you&#8217;re experiencing. </p>
<p>An alternative to Advair is Symbicort. It comes in a variety of strengths, just as Advair does. The advantage to Symbicort is that it has been approved (in Canada and Europe, I don&#8217;t know about the US) for use as a rescue inhaler too. In effect, all you would be using is Symbicort. There have been studies showing that the Pulmicort (inhaled steroid) in Symbicort helps to reduce the severity of  an asthma attack over time. This happens because, the patient takes their Symbicort for an asthma attack, then is fine. The dose of inhaled steroid they just took helps prevent further asthma attacks. I like the idea of it. I feel that Symbicort as a long-acting controller and as a rescue inhaler will revolutionize asthma treatment like Advair did when it came out. There is a possibility that asthmatics might only need one inhaler for all purposes. That&#8217;s a lot simpler than knowing how to use an Advair diskus and an Albuterol inhaler. Less patient education is then required for them to be able to use their devices properly. Advair is great though, too. I&#8217;m not discounting it at all. It just isn&#8217;t meant to be used during an asthma attack under any circumstances.</p>

<p><b>A: </b>I suppose if you had a medicine triggered asthma attack, it would probably happen right after you inhaled the medicine.I&#8217;m pretty sure you have to be allergic to the medicine for it to trigger an asthma attack, so that would mean, you would probably notice an allergic reaction(i.e. the asthma attack) within the first few uses. So that means, if you&#8217;ve been using the medicine for a while, chances are you are probably not going to have an attack because of the medicine. Regardless, if you do have an really bad asthma attack, you can probably discuss with your doctor what triggered it.</p>
